Individual Payment Request Form
Use this form to request payments to an individual in exchange for the individual's professional services, or honoraria/guest speakers.
Policies:
- Payment requests must be submitted to AASC prior to receiving any services from the individuals.
- Examples of the individuals mentioned here include, but are not limited to consultants, guest speakers, guest lecturers, freelancers, artists, graphic designers, copyeditors, writers, translators, videographers, etc.
- Examples of the individual professional services mentioned here include, but are not limited to professional consultation, guest speaking, writing, copyediting, videography, translation, graphic design, etc.
